Overview
claude-reflect is a Claude Code plugin that uses hooks to queue corrections you make during sessions, then writes approved ones to CLAUDE.md (global, project, subdirectory) and AGENTS.md via /reflect. /reflect-skills scans session history for repeated requests and drafts slash-command files in .claude/commands/. A separate reflect-mod variant checks each short prompt with a model and shows a save prompt in the session.

Strengths
- Human review step: nothing is written to CLAUDE.md without approval
- Syncs to CLAUDE.md, subdirectory CLAUDE.md files, skill files and AGENTS.md (Codex, Cursor, Aider)
- /reflect --scan-history recovers corrections from past sessions; --dedupe merges similar entries
- Runs on macOS, Linux and Windows with Python 3.6+; 332 tests reported
Weaknesses
- Plugin capture uses regex; multi-language detection only happens at /reflect time
- Depends on Claude Code session history, which is deleted after 30 days by default
- Mod and plugin must not be installed together or corrections are captured twice
- Mod is early access and needs Claude Code 2.1.286+ and a model call per prompt
